The State of Wisconsin offers a number of health benefits programs for eligible children, individuals, and families. For eligibility information, call Medicaid Enrollment Services at (800) 291-2002. 

BadgerCare Plus

For pregnant women, children, and families who meet eligibility requirements. This plan has two tiers, depending on income. 

Family Planning Only Services

Family planning and reproductive health care coverage for individuals age 15 and older.  

Elderly, Blind or Disabled Plans (EBD)

Medicaid for people determined blind or disabled by the Disability Determination Bureau (DDB) or who are at least 65 years old. 

Wisconsin Medicaid and BadgerCare Plus Emergency Services Plan

For people experiencing a medical emergency who otherwise cannot get Medicaid because of their citizenship or immigration status.

Provides mammograms and pap smears. To apply, call to schedule an appointment. 

Health Insurance Risk-Sharing Plan (HIRSP): (800) 822-4777 
Health insurance to Wisconsin residents, under 65, who are unable to find adequate health insurance due to medical conditions or who have lost their group health insurance. Applicants are required to meet HIRSP eligibility criteria to qualify.

Wisconsin Medicaid HMO Assistance 
Most Medicaid plans require you to enroll in an HMO. Here is some information that may assist you. 
Applying for Benefits Online: Apply at access.wi.gov – “Apply for Benefits” 
Mail: Call MiLES (Milwaukee Enrollment Services) at (888) 947-6583 to be sent a paper application. 
In Person: Go to Coggs Center – 1220 W. Vliet St., Milwaukee, WI 53205
